To receive a referral form, contact either office and request a form. We will send to your .gov email address. You can also download the form below. Once submitted referrals are started within 24-48 hours and are typically scheduled within the following week provided all parties schedules align.
For Private Pay/Family Matters
To receive a referral, contact either office. We will send a referral form to your email, Or you can stop by either office to pick one up. Or you can download the form below. The custodial parent would need to fill out a referral form as well. Once we receive the referral forms a non-refundable deposit of $75.00 needs to be paid before any scheduling can be done. Our rates are $50.00 per hour. Payment is due at the time of visit, with the option to prepay.
